It usually takes 3 to 5 months to see a dermatologist in Lane County Oregon due to the shortage of specialists. When patients finally are able to be seen at the dermatology clinic for their appointment, they disappointingly find out that the dermatologist do not practice or perform aesthetics procedures. When patients go to a cosmetic or plastic surgery practice, most of the time they are not allowed by the staff to see the surgeon unless they want surgery. They are left to see their assistants. Patients are left with no choice but to proceed with treatment with less qualified individuals either in that practice or elsewhere. This often leads to poor outcomes and unsatisfactory results. When complications arise, these are often not addressed by the assistants because not only for the lack of training but also because they are not licensed to manage or treat medical complications. Medical doctors are the only professionals (besides NP’s and PA’s) licensed to diagnose and treat medical conditions. Worst case scenario, the patient is left but no choice to go to the emergency room with embarrassment and outstanding medical bills. Unbeknownst to many, facial fillers can cause blindness and tissue death if accidentally injected into an artery. Some emergency rooms do not stock medicines like hyaluronidase to dissolve the fillers. Due to the reasons above, Reverza Aesthetic Medicine was founded by Dr. Asirot. Dr. Asirot did not take a weekend course to learn aesthetics. During medical school and residency, his electives were mostly in plastic surgery and dermatology. Yearly he attends training, preceptorships and conferences in skincare and aesthetics. He also deals with skin diseases daily in his medical practice performing skin exams, diagnosing and treating skin conditions and removing skin lesions such as skin cancer. However, at Reverza Aesthetic Medicine we only offer aesthetic treatments to fill the gap in the community. To assure optimal results, Dr. Asirot performs the procedures himself. Other practices only use the “name” of the doctor for licensing purposes but is never present at the medspa or practice.

Beauty Tips You’ll Be Thankful For

by Dr. Asirot

A good skin care routine is important no matter what the season is. Thanksgiving is a festive yet stressful season to celebrate with friends and family. It is easy to indulge in pies and sugary food. Skipping a step in your skincare regimen can lead to skin breakouts and unfavorable consequences.

Cleanse. Wash away the dead skin cells and impurities of your skin using a gentle facial cleanser. Don’t sleep with makeup on as clogged pores may lead to acne.

Rejuvenate. It is advisable to apply corrective serums and/or medications after patting your face dry. Antioxidant serums neutralize free radicals that cause skin damage. Applying a retinoid such as retinol improves the appearance of fine lines, pores, and skin texture. Products that target melanin production help with skin darkening or hyperpigmentation.

Moisturize. It is important to hydrate your skin. Cold months cause more skin dryness. Water-based, hypoallergenic moisturizers are generally appropriate for sensitive skin. Products which contain hyaluronic acid help bind moisture to your skin giving your skin a smoother appearance.

Protect. Use broad spectrum sunscreen that protects against UVA and UVB rays with at least SPF30. Reapplication every couple of hours even during cloudy days prevents wrinkles and skin cancer.

Happy thanksgiving from Dr. Asirot!

(Excerpt, Q&A with Dr. A) By Sylvia Pratt, Medical Correspondent

Why did you decide to go into aesthetic medicine?

I went to aesthetics because I’ve always been passionate about making things around me beautiful. I am an artist, a self-proclaimed one (chuckles)—probably of the Renaissance era since I want to make things beautiful. On a serious note, I’ve always been interested in the aging process and wanted to discover ways to delay it. I initially thought that aesthetic procedures will help me fund my reverse aging research but I was quite wrong. There is huge overhead in this type of endeavor and margins are slim unlike what majority of people surmise. You have to be really passionate about making people love themselves externally so they can love their inner selves better.

What made you interested in studying hair?

Regenerative medicine is a relatively new field and its application in skin and hair rejuvenation excites me. By drawing a patient’s own blood and harvesting PRP (platelet rich plasma) one can stimulate hair growth by applying and injecting it into the scalp. Hair looks more natural that way compared to the conventional hair transplant.

My dermatologist friends have skewed away from doing hair fellowships and further studies in hair because they do not want to be associated with the salon industry, and I get that. Majority of them are not doing aesthetics not only because they are not trained to do it but also for a variety of reasons. Some assumptions include aesthetics cheapening the profession, and the difficulty in managing expectations of patients for these elective procedures.

Wait, are you saying that some dermatologists are not trained to do aesthetics like Botox and fillers?

Dermatologists are trained in treating skin conditions and diseases like psoriasis, acne and skin cancer. I was fortunate to be trained by the dermatology faculty for several months during my residency. Interestingly, majority of them do not practice aesthetics. Cosmetic procedures are not a requisite to graduate in dermatology [residency]. Cosmetic procedures are also not confined to dermatologists and plastic surgeons unlike public perception. Plastic surgeons are formally trained in doing surgeries such as skin grafts for burn and some breast reconstruction. A medical doctor has to be incredibly devoted in learning about non-surgical and minimally invasive aesthetics if one wants to pursue that route. Of course, it also requires someone with artistic eye and dexterity to deliver good results. Not all doctors can and should practice aesthetics.

Walk us through your journey with aesthetic medicine.

I’ve always wanted to do aesthetics after college. The field inspired me to survive the hardships in medical school…that towards the end I will be able to combine art and medicine in my practice. I did all of my electives in plastic surgery during medical school but the economic recession hit and many people dissuaded me to pursue the specialty at that time. Several plastic surgery offices closed down leaving the surgeons scrambling to find alternative careers and jobs in wound care.

Before, during and after residency, I’ve attended several trainings and conferences in dermatologic and surgical aesthetics, hoping one day I will be able to build my own practice. I’ve been fortunate as well to find several mentors in the trade which is key. The confidence I help my patients regain after I perform cosmetic treatments gives me great satisfaction, and that drives me to do better each day as I improve my craft.

Sunscreens not only decrease the risk of getting skin cancer, but it also delays premature aging. Below you will find an image of a man published on the New England Journal of Medicine of a trucker with wrinkled face on the left side as a result of sun damage. The harmful ultraviolet rays from the sun hit the left side of the face for 25 years resulting in thickening of the skin, loss of elasticity and wrinkling. The picture published in 2012 is so powerful that I have to share it with you.

When I did a systemic literature review on sunscreen use years ago, I found out that certain people are staying longer under the sun after sunscreen use (which I advise against). While sunscreens offer some protection from the harmful UV rays, they do not entirely protect you. You must also minimize your exposure to the sun if you cannot avoid it as much as possible. If you have sensitive skin like me, try non-comedogenic sunscreens that are water-based because they won’t clog your pores as much. I also use sunscreens that are benzone-free which do not harm corals and marine life.

As a gerontologist, I take care of patients with complex medical problems. A lot of times I hear them say, “Don’t get old doc!” I empathized with their suffering as I am starting to feel the toll of aging myself. I have been giving lectures and publishing articles on aging, preventive medicine and dermatology. The desire to help people and my insatiable curiosity to discover solutions to slow the aging process drove me to establish Reverza. Reverse aging goes beneath the surface. Looking and feeling young are more than skin deep and require good lifestyle choices. My main focus at this stage is skincare, aesthetics and regenerative medicine. As we go through this journey, I wanted to share my knowledge and secrets (that they do not teach in medical school), so we can slowly age gracefully and beautifully.
